This teleconsultaion will be completed via telephone only. Video not required. In order to approve session date and time, provider must receive Patient Information Form, Patient Information Form, Consent for Treatment, Notice of Privacy Practice, Tele-consultation Consent form and Terms and Conditions signed by patient. Forms will be provided to patient upon appointment request. If forms have been submitted in the past, do not resubmit unless stated otherwise. Provider will review patient medical records to ensure forms are available to provider. Once forms are reviewed, appointment date and time will be approved by provider. Cancellations or rescheduling must be made at least 24 hours before the appointment. Failure to do so will result in a 20% charge of their total appointment cost. Repeated no-shows may lead to dismissal.
