Cancellation Policy

NOTE: There is a travel fee associated with all testing done in patient homes. In order to approve session date and time, provider must receive Consent for Treatment, Notice of Privacy Practice and Terms and Conditions signed by patient. Forms will be provided to patient upon appointment request. If forms have been submitted in the past, do not resubmit unless stated otherwise. Provider will review patient medical records to ensure forms are available to provider. Once forms are reviewed, appointment date will be approved by provider. Patient will receive a Patient Information form that must be submitted to provider 24 hours prior to appointment date.
