1. Scope of Services
THE SALUTARY FAMILY MEDICINE provides family medicine services, including but not limited to preventive care, diagnosis, treatment, and management of acute and chronic medical conditions.
2. Appointments and Cancellations

Patients are required to schedule appointments in advance.

Cancellations or rescheduling must be made at least 24 hours before the appointment. Failure to do so may result in a cancellation fee.

Repeated no-shows may lead to dismissal from the practice.
3. Insurance and Payments

We DO NOT accept insurance plans at this time.

Patients are responsible for full payment prior to time of service.
4. Privacy and Confidentiality

We are committed to protecting patient privacy in accordance with HIPAA regulations.

Medical records will not be shared without patient consent, except as required by law.
5. Prescription Policy

Medication refills require a prior consultation or approval from a provider.

Controlled substance prescriptions may require periodic evaluations and will not be refilled without proper documentation.
6. Patient Responsibilities

Patients must provide accurate and complete medical history.

Patients must adhere to prescribed treatment plans and follow-up appointments.

Disruptive or inappropriate behavior may result in termination of care at our practice.
7. Emergency Situations

Our practice does not provide emergency services. In case of a medical emergency, call 911 or go to the nearest emergency room.
8. Changes to Terms and Conditions

We reserve the right to update these terms and conditions at any time. Patients will be notified of any significant changes.
